TERMS AND CONDITIONS of this STALLION SERVICE APPLICATION
1.   This mare is booked specifically by name. The booking is valid to the mare named and may not be transferred without consent of Seelster Farms Inc.
 
2. Mare owner(s) agree(s) that the mare named in this application shall be healthy and in sound breeding condition. Seelster Farms Inc. reserves the right to reject any mare not in good physical condition, or blind, or vicious and unmanageable. This mare must be registered with the Standardbred Canada and/or the United States Trotting Association.
 
3. Seelster Farms Inc., its owners, members, employees, or agents shall not be liable or responsible in any manner for any accident or injury, disease or disability suffered by the mare (or her foal). The mare owner acknowledges that the mare (and foal) will be on Seelster Farms Inc.'s premises entirely at the mare owner's risk, and waives any and all claims resulting from such injury, disease or disability.
 
4. The service fee shall be due when the mare produces a live foal (a foal that can stand up alone and nurse) or when the mare changes ownership, whichever occurs first. The Federal 6% Goods and Services Tax will be applicable to service fees, boarding and veterinary charges, etc., and will be added to all invoices. Seelster Farms Inc.'s G.S.T. number is R104773304.
 
5. All accounts for board, veterinary fees, service fees (if due), overdue interest and expenditures made on behalf of the mare (and/or her foal) will be settled upon receipt of duly rendered statements, and, in any event, before the mare is removed from the farm. A service charge of 18% per annum (1.50% per month) will be charged on all accounts unpaid after thirty days. Seelster Farms Inc. may refuse release of the mare until the accounts are settled and will not issue a mating certificate to mare's owner(s) with an unpaid account.
 
6. For mares booked to farm stallions, the board rate will be $15.00 per day ($2.00 per day extra for a pregnant mare or for a nursing foal). For mares booked to outside stallions, the board rate will be $15.00 per day ($2.00 per day extra for a pregnant mare or for a nursing foal). The breeding season commences on February 15, 2008 and closes on July 11, 2008.
 
7. The mare must have a NEGATIVE COGGINS TEST for equine infectious anemia dated no earlier than twenty-four (24) months prior to the date of her arrival at the farm. A certificate to this effect MUST ACCOMPANY THE MARE upon her arrival at the farm. Seelster Farms Inc reserves the right to retest the mare upon her arrival at the owner's expense.
 
8. Semen transportation is available WITH PRIOR NOTIFICATION AND APPROVAL from Seelster Farms Inc. (Complete the box on the front of the application and below to apply.)
 
9. Seelster Farms Inc. encourages mare owner(s) to have the mare's (and foal's) vaccinations and de-worming up-todate prior to her (their) arrival at the farm. The owner(s) shall provide an up-to-date General Health record for the mare (and foal) on the Background Information form returned with each approved Stallion Application.
